Comments:

An unannounced renewal inspection was conducted by the Licensing Specialist on 11/20/19 from 9:58 a.m. to 4:36 p.m. at the corporate office in Richmond, where the staff records are maintained, for the purpose of reviewing staff records for this facility. Also, an unannounced renewal inspection at the facility was completed by the Licensing Specialist on 11/21/19 from 11:01 a.m. to 2:42 p.m. The current census is seven (7) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. Four staff records were reviewed, which consist of the new staff members hired since the January 2019 inspection. Background checks for the Board Officers were previously reviewed during this fiscal year. On 11/20/19, the Human Resources Compliance and Training Analyst was available and accessible during the inspection. An exit meeting was conducted with the Executive Director (accessible by video conferencing), the Human Resources Compliance and Training Analyst, and the Director of Human Resources and Talent Management at approximately 4:36 p.m. The Acknowledgment of Inspection form was signed and left at the corporate office. On 11/21/19, the Regional Supervisor was available and accessible during the inspection. An exit meeting was conducted with the Regional Supervisor and the Senior Life Skills Coach at approximately 2:00 p.m. The Acknowledgment of Inspection form was signed and left at the facility. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website within 5 business days.
