A non-mandated inspection was initiated on 8/25/21 and concluded on 9/1/21. A self-reported incident was received by the department on 8/4/21. This inspection focused on intake and admission. The Executive Director was contacted by telephone on 8/25/21 to conduct the inspection. The licensing inspector emailed the Executive Director a list of documentation required to complete the inspection. The Executive Director reported that the current census was six (6) youth.
