Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 11/16/23 from 11:09 AM ? 4:44 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A self-reported incident was received by VDSS Division of Licensing on 10/17/23 regarding allegations in the area(s) of: maintain supervision over the independent living arrangement. Number of children in care: 3 Number of youth records reviewed: 1 Number of staff records reviewed: 2 Number of interviews conducted with staff: 5 Additional Comments/Discussion: An entrance conference for this agency was conducted with the Assistant Director on 11/16/23. A youth?s electronic record was reviewed on this date. The Assistant Director was available and accessible during the inspection. A preliminary findings meeting was held with the Assistant Director. The Executive Director participated in this meeting by phone. Staff records were reviewed at the corporate office in Richmond on 11/28/23. The HR Business Partner was available and accessible during this inspection. A preliminary findings meeting was held with the HR Business Partner. An exit meeting was conducted on 12/27/23 to review the inspection findings. The evidence gathered during the investigation did not support the self-report of non-compliance with standard(s) or law.
