An unannounced complaint inspection was conducted on 7/16/19 from approximately 10:58 a.m. to 4:05 p.m. This inspection occurred due to an allegation involving a youth placed in an independent living arrangement. During the inspection, the census was 12. Violations were not cited. Therefore, this complaint it determined invalid.
The following is a listing of the activities for this inspection:
One youth record was reviewed. Three staff were interviewed.
On 7/16/19, the Regional Director and the Program Manager were on site and accessible during the inspection. The Licensing Specialist interviewed these two staff members at the beginning and towards the end of the inspection in order to seek clarification about the allegation. An Acknowledgement of Inspection form was signed and left at the facility.
The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website within 5 business days.
