Comments:

An unannounced renewal inspection was completed by the Licensing Specialist on 12/19/19 from 10:17 a.m. to 2:22 p.m. at the corporate office in Richmond, where the staff records are maintained, for the purpose of reviewing staff records for this facility. An unannounced renewal inspection was conducted by the Licensing Specialist on 12/20/19 from 1:38 p.m. to 3:47 p.m. at the facility. The current census is nine (9) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. Two staff records were reviewed, which consist of the newest staff members hired since the May 2019 monitoring inspection. Background checks for the Board Officers were previously reviewed during this fiscal year. On 12/19/19, the Human Resources Compliance and Training Analyst were available and accessible during the inspection. An exit meeting was conducted with the Program Development Specialist and the Human Resources Compliance and Training Analyst at approximately 2:05 p.m. The Acknowledgment of Inspection form was signed and left at the corporate office. On 12/20/19, the Regional Supervisor was available during the inspection. An exit meeting was conducted with the Regional Supervisor and the Assistant Director (accessible by video conferencing) at approximately 3:28p.m. The Acknowledgment of Inspection form was signed and left at the facility. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website.
