Comments:

This inspection was conducted by the licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 1/8/21 and concluded on 1/29/21. The Executive Director was contacted by email on 1/8/21 to initiate the inspection. The Executive Director reported that the current census was seven (7) youth. The Licensing Specialist emailed the Executive Director a list of items required to complete the inspection. The following is a listing of the activities for this inspection: Reviewed one current resident record. Nine personnel records were reviewed, which consist of the new staff members hired since the December 2019 inspection. The Human Resources Compliance and Training Analyst was interviewed on 1/19/21 and 1/21/21. The Executive Director was interviewed on 1/21/21. Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.

Standard #:	22VAC40-191-50-A-1-a
Description:	Violation: Based on interviews and review of staff?s, S3?s, background checks, the facility failed to require documentation of satisfactory background checks. The sworn statement or affirmation for S3 was not fully completed. Findings: 1) Question 2 on the sworn statement or affirmation states the following: ?Have you ever been convicted of or are you the subject of pending charges of any crime outside the Commonwealth of Virginia?? 2) S3 selected or checked the following box beneath question 2 ? ?Yes (pending outside Virginia).? Beneath the check box there is a sentence that states the following: ?If yes to convicted or pending, specify crime(s) and state, or other location:? S3 did not specify crime(s) and state, or other location as directed. 3) S11 was interviewed in regard to S3?s incomplete Sworn Statement. S11 stated the following, ?His name search and Fingerprint results did not indicate any pending charges so there was no further follow-up to my knowledge.?
Plan of Correction:	The Executive Director will ensure that moving forward the Human resources department will ensure that the candidate is not allowed to move through the hiring process if the additional information required on the form is not provided. Additionally, we will request supplemental information from the candidate to ensure alignment with the results of the FBI and VSP background search.
