Comments:

A renewal inspection was initiated on 11/22/21 and concluded on 12/09/2021. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the current census was six (6) youth. The Licensing Specialist emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The following is a listing of the activities for this inspection: Reviewed one current resident record. Five personnel records, which consist of new staff, were reviewed. The background checks for two Board Officers were recently reviewed for another location. The Manager of Compliance and Development and the Assistant Director were interviewed together on 11/30/21. Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.

Description:	Violation: Based on interviews and review of staff?s, S1?s, background checks, the facility failed to require documentation of satisfactory background checks. The sworn statement or affirmation for S1 was not fully completed. Findings: 1) There are four questions on the sworn statement or affirmation. Each question requires a response. 2) S1 did not respond to the second question listed on the sworn statement or affirmation. 3) S7 was interviewed in regard to S1?s sworn statement or affirmation. S7 acknowledged the form was not fully completed.
