Comments:

An unannounced monitoring inspection was conducted by the Licensing Specialist on 3/15/22 from 10:30 p.m. to 1:24 p.m. at the agency. Also, an unannounced monitoring inspection was completed by the Licensing Specialist on 3/17/22 from 10:12 a.m. to 3:12 p.m. at the corporate office in Richmond, where the staff records are maintained, for the purpose of reviewing staff records for this agency. The current census is six (6) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. Two staff records were reviewed. On 3/15/22, the Executive Director and the Assistant Director were available and accessible during the inspection. Both were interviewed on 3/24/22 in order to provide clarification about several documents reviewed. The Acknowledgment of Inspection form was signed and left at the agency. On 3/17/22, the Manager of Compliance and Development was available and accessible during the inspection. This staff member was interviewed on the same date about documents within the personnel records. The Acknowledgment of Inspection form was signed and left at the agency. Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.

Description:	Based upon an interview and review of staff?s, S2?s, personnel record, the agency failed to include the date of separation for a position held with the licensee in the record. Findings: 1) Staff, S2, initially began employment with this agency at another location in October 2021 as a ?Full Time Life Skills Coach Trainee.? This month, S2 started a new position, ?Senior Life Skills Coach Trainee,? at this location. 2) Upon review of S2?s personnel record, the date of separation from the Full Time Life Skills Coach Trainee position was not in the personnel record for this staff person. 3) S5 was interviewed about S2?s date of separation. 4) S5 acknowledged that S2?s separation date was not in S2?s record.
