Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection:
12/8/22 from 10:20 AM ? 2:59 PM and 12/9/22 from 12:29 PM ? 4:12 PM

The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.

Number of youth in care: 8
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Number of interviews conducted with staff: 3
Additional Comments/Discussion:
An entrance conference was held at the agency on 12/9/22. Also, on the same date, a youth?s record was reviewed at the agency. The Senior Life Skills Coach Trainee was available and accessible during the inspection. The Executive Director and the Senior Life Skills Coach Trainee were interviewed on the same date to address questions about two independent living arrangement (ILA) settings. Staff records were reviewed at the corporate office in Richmond on 12/8/22. The HR Business Partner was available and accessible during the inspection. The HR Business Partner was interviewed on the same date about several documents reviewed. The Executive Director was interviewed on 12/22/22 to address additional questions about the ILA settings. An exit meeting was conducted on 12/30/22 to review the inspection findings.

The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.

The department's inspection findings are subject to public disclosure.
