Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 2/21/23 from 11:44 AM ? 6:13 PM ? Inspection conducted along with the complaint inspection for YouthQuest-Charlottesville 5/18/23 from 2:04 PM ? 3:00 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Complaint was received by VDSS Division of Licensing on 1/18/23 regarding allegations in the area(s) of: Independent Living Arrangements. Number of youth in care: 9 Number of youth records reviewed: 0 Number of interviews conducted with staff: 3 Additional Comments/Discussion: On 2/21/23, an entrance conference was held with the Executive Director. During this meeting, policies and procedures were requested. The Executive Director was available and accessible during the inspection on 2/21/23. On 5/18/23, the Executive Director and the Program Manager were available and accessible during the inspection. The following staff were interviewed: Executive Director on 2/21/23 and 5/18/23; Assistant Director on 5/18/23; and the Program Manager on 5/18/23. This agency?s policies and procedures were reviewed on 5/18/23. An exit meeting was conducted on 6/6/23 to review the inspection findings. The evidence gathered during the investigation did not support the allegations of non-compliance with standard(s) or law.
