Comments:

An unannounced monitoring inspection was conducted by the Licensing Specialist on 7/24/19 from 10:40 a.m. to 3:34 p.m. at the corporate office in Richmond, where the staff records are maintained, for the purpose of reviewing staff records for this facility. Also, an unannounced monitoring inspection at the facility was completed by the Licensing Specialist on 7/31/19 from 11:40 a.m. to 4:30 p.m. The current census is ten (10) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. Six staff records were reviewed, which includes the three new staff members hired since the November 2018 inspection. On 7/24/19, the Executive Director and the Human Resources Compliance and Training Analyst were available and accessible during the inspection. An exit meeting was conducted with the Executive Director and the Human Resources Compliance and Training Analyst on the previously mentioned date at approximately 3:14 p.m. The Acknowledgement of Inspection form was signed and left at the corporate office. On 7/30/19, the Program Manager was available and accessible during the inspection. The Program Manager was interviewed during the inspection to seek clarification about the youth?s placement and the documents reviewed. An exit meeting was conducted with the Program Manager, Regional Supervisor (accessible by video conferencing), and the Program Development Specialist (accessible by video conferencing) on the previously mentioned date at approximately 3:41 p.m. The Acknowledgement of Inspection form was signed and left at the facility. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website within 5 business days.
