Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced inspection was initiated on 11/24/2020 in response to a complaint related to staffing patterns and policies, physical environment within the Independent Living Arrangement, and implementation of the agency?s policies and procedures for resident medication administration. The inspection concluded 12/9/2020.

The Director of Independent Living was contacted by telephone and email. The inspector reviewed one (1) youth and one (1) foster child record.

There was one citation for a violation of the Standards for Licensed Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on record reviews for Youth 1 (Y1) and Foster Child 1 (FC1) and interview, the agency failed to obtain and place in the records for Y1 and FC1, documentation required by the agency's policies and procedures.

Findings:

1) The agency's policies and procedures state "YouthQuest will maintain within the youth's electronic record a list of all medication the youth is currently taking. The chart will also contain written documentation from the youth's physician a statement addressing the youth's ability to administer their medication independently under staff supervision."

2) Staff 1 (S1) was interviewed regarding the agency's policies and procedures. S1 indicated the statement is located in medical notes from the medical provider and agreed to send the Licensing Inspector the statements for Y1 and FC1.

3) Following the exit interview, (S1) communicated to the Inspector that the medical notes were reviewed and the statement is no longer included. S1 reports contacting the medical provider to discuss. The medical provider reviewed and informed that the statement is no longer included since a change in computer systems was implemented. The medical provider informed that the issue would be corrected.

Plan of Correction: 22VAC40-141-(2)-40-B
Executive Director will ensure that appropriate documentation is in the resident file pertaining to medication administration. Executive Director has made adjustments to current policy to include "for youth under the age of 18, the chart will also contain written documentation from the youth's physician or assistant physician a statement addressing the youth's ability to administer their medications independently under staff supervision." Executive Director has ensured that the medical provider "TrueNorth" will include written awareness in their notes to meet theYouthQuest policy.
