Comments:
An unannounced monitoring inspection was conducted by the Licensing Specialist on 1/25/22 from 3:59 p.m. to 5:31p.m. for the purpose of reviewing a youth record. Also, an unannounced monitoring inspection was completed by the Licensing Specialist on 1/28/22 from 12:33 p.m. to 4:02 p.m. for the purpose of reviewing staff records. The current census is five (5) youth.

The following is a listing of the activities for this inspection:
Reviewed one current youth record. Two personnel records were reviewed, which includes the newest staff member hired since the October 2021 inspection. The Executive Director and the Regional Supervisor were interviewed on 1/28/22. The Manager of Compliance and Development was interviewed on 1/31/22.

Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.

Standard #: 22VAC40-131-40-B
Description: Based upon an interview and review of staff?s, S1?s, personnel record, the agency failed to comply with their own policies and procedures.
Findings:
1) The job description for the Evening Support Specialist states the following as it pertains to ?Education/Experience? ? ?The Evening Support Specialist shall have a baccalaureate degree or a high school diploma with at least one to two years experience working with children and/or families.?
2) S1 has occupied the Evening Support Specialist position for 19 days.
3) S1?s personnel record verifies this staff member is a high school graduate.
4) S1?s resume shows that S1 does not have at least one to two years experience working with children and/or families, which is required for a high school graduate.
