Comments:

An unannounced monitoring inspection was completed on December 17, 2019 from 9:05 a.m. to 2:05 p.m. The licensee reports thirteen (13) children in placement and eleven (11) approved provider homes. During this inspection two (2) foster child and five (5) foster home records were reviewed. The Foster Care Site Leader/Supervisor participated in the entrance conference, was available for the inspection, and participated in the exit interview. A caseworker participated in the entrance conference. The Regional Director participated in the exit interview by phone. The exit interview was conducted from 1:35 p.m. to 2:05 p.m. An Acknowledgement form was signed and left with the licensee at the end of the inspection. There was one(1) citation for a violation of the Standards for Licensed Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed. The provider?s response for the ?plan of correction? was not received as of 12/30/2019 and will not appear on this Violation Notice.

Description:	Violation: Based on record review, the agency failed to consider all information gathered in the home study process prior to making a decision on foster homes 1 (FH1). Findings: The case record for FH1 contained one completed reference form. The home study noted the names of three references and to refer to the file for the original references obtained. The home study narrative did not include additional information to explain what information was obtained from the references and how it was considered in approving the home. The finding was discussed with the Site Leader/Foster Care Supervisor (S1) during the exit interview. S1 located a second reference in a binder in the office. A third reference was not located. S1 acknowledged the missing information.
