Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced renewal inspection was conducted on 1/25/2021 and 1/26/2021. The licensee reported 15 children in foster care and 11 approved foster homes. During this inspection two (2) foster child and two (2) foster home records were reviewed.

The Regional Manager for Foster Care and Independent Living responded to the notice of inspection, provided access to case records, and remained available during inspection. An exit interview was conducted with the Regional Manager for Foster Care and Independent Living, Foster Care Supervisor, and Foster Care Specialist on 1/26/2021.

There was one (1) citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review, the agency failed to obtain tuberculosis (TB) screening results within 13 months of the re-evaluation of Foster Home 1 (FH1) for all individuals in the household.

Findings:
1) The re-evaluation addendum for FH1 was completed on 12/1/2020.
2) The re-evaluation addendum documented the household included four children who were not in foster care placement
3) The re-evaluation addendum documented TB screening/test results were obtain for one (1) child 14 months prior to the re-approval
4) The re-evaluation addendum documented TB screening/test results were obtained for three (3) children 19 months prior to the re-approval
5) The findings were discussed during the exit interview. Staff 1, 2, and 3 reviewed the
record and did not locate TB screening/test results within 13 months of the re-approval
for the four (4) children and acknowledged the findings.

Plan of Correction: TB screening results will be completed on all family members and in the chart within 30 days of this notice.
