Comments:

A monitoring inspection was initiated on 11/3/2021and concluded on 11/5/2021. The Director of Foster Care and Independent Living was contacted by telephone to initiate the inspection and reported that the current census was ten (10). The inspector emailed the Director of Foster Care and Independent Living a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) foster child, three (3) staff, and two (2) provider home records. An exit interview was conducted with the Director of Foster Care and Independent Living, Regional Manager of Foster Care, and Foster Care Supervisor on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Violation Based on record review, the agency failed to address required elements in the Individualized Service and Treatment Plan (ISTP) for Foster Child 2 (FC2). Findings: 1) The ISTP for FC2 did not include criteria for achievement or a target date for the goal. 2) The ISTP for FC2 did not include criteria for achievement or target dates for the objectives. 3) The findings were discussed during the exit interview and staff acknowledged the findings.
