Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced mandated monitoring inspection was initiated on September 28, 2020 and concluded on September 30, 2020. The Program Director was contacted by telephone to initiate the inspection. The agency reports 24 foster children in placement and 17 approved provider homes. There are no new staff hires since the last inspection. The inspector emailed the Program Director a list of items required to complete the inspection. The inspector reviewed two (2) provider home records and three (3) foster child records. An exit interview to discuss findings was held with the Program Director by phone at the conclusion of the inspection.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The plan of correction should include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Description: Violation:
Based on a review of the social histories for FC2 and FC3 and interview with personnel (S1), the agency failed to include required elements in the social history.

Evidence:
1. The social history dated 08/31/2020 for FC2 failed to address the following required elements: 260.B.8 - Education and occupation of the child's aunts, uncles, and grandparents and 260.B.9 - Medical and psyciatric history of the child's aunts, uncles, and grandparents.
2. The social history dated 03/20/20 for FC3 failed to address the following required element: 260.B.6 - Child's developmental history.
3. During the exit interview, S1 acknowledged that the information was not included in the social histories of FC2 and FC3.

Plan of Correction: All TFC staff will be trained on the regulations in question. All Social Histories will be reviewed by the Program Manager to ensure compliance with the cited regulations going forward.
