Technical Assistance:
Licensing Specialist provided technical assistance around 22VAC40-131-200.G.1: how to document type of family home approval when the family is approved as a provider for multiple types and how to document if the type of approval changes after original approval and issuance of certificate of approval.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced mandated monitoring inspection was initiated on August 12, 2020 and concluded on August 19, 2020. The Director of Operations was contacted by telephone to initiate the inspection. The agency reports 32 foster children in placement and 24 approved provider homes. There was one new staff hire since the last inspection. The inspector emailed the Director of Operations a list of items required to complete the inspection. The inspector reviewed three (3) provider home records, four (4) foster child records, and two (2) personnel records. An exit interview to discuss findings was held with Director of Operations and the Clinical Supervisor by phone at the conclusion of the inspection.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The plan of correction should include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Description: Violation:
Based on the review of the foster child records (FC1, FC2, FC3) and based on an interview with personnel (S1, S2) the social history did not include an assessment of the child's developmental history.

Evidence:
(1) The social history for FC1 dated 02/20/2020 did not include an assessment of FC1's developmental history.
(2) The social history for FC2 dated 05/03/2019 did not include an assessment of FC2's developmental history.
(3) The social history for FC3 dated 02/24/2020 did not include an assessment of FC3's developmental history.
(4) During an interview, S1 and S2 carefully reviewed the social histories for FC1, FC2, and FC3. S1 and S2 acknowledged that the social histories for FC1, FC2, and FC3 did not an assessment of developmental history.

Plan of Correction: CCC will include assessments of the developmental history on all social histories for any child placed with this agency. All applicable staff will be trained to ensure compliance with this requirement. Clinical Supervisor will continue to monitor and review files for adherence to this standard.
