Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on January 20, 2021 and concluded on March 9, 2021. The Director of Operations was contacted to initiate the inspection. The agency reported 26 approved homes and 37 foster children in placement. There were no new staff since the last inspection. The inspector emailed the Director of Operations a list of items required to complete the inspection. The inspector reviewed five (5) provider home records, four (4) foster child records, and records for four (4) board officers to include background checks and references for three new appointees to ensure documentation was complete.

An exit interview to discuss findings was held with the Director of Children's Services, Director of Operations, and Clinical Supervisor at the conclusion of the inspection.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The plan of correction should include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Description: Violation:
Based on a review of the social history for a foster child (FC1) dated 11/20/2020 and interview with personnel (S1 and S2), the agency did not include all required elements in the social history

Evidence:
1. The social history for FC1 did not include the following elements: 260.B.8: education and occupation of the child's aunts, uncles, and grandparents and 260.B.9: medical and psychiatric history of the child's aunts, uncles, and grandparents.
2. During an interview with S1 and S2, it was acknowledged that these elements were not included in the social history.

Plan of Correction: CCC will monitor and review all Social Histories for each child prior to filing. All applicable staff will be trained to ensure all applicable fields are completed thoroughly to ensure compliance with this standard. Program Manager and Clinical Supervisor will review records to ensure all Social Histories are in compliance.
