The Licensing Inspector conducted an unannounced complaint investigation in response to a complaint that was received by the licensing office on October 11, 2019. The complaint investigation was conducted at the same time as an unannounced monitoring inspection from 9:19 a.m. to 4:15 p.m. on October 22, 2019. Exit meeting held via telephone on November 1, 2019 with the Executive Director. The complaint investigation occurred due to an allegation regarding youth placed in independent living arrangements. Records were reviewed, other documentation reviewed, and interviews were conducted with staff and a resident. Information gathered during the investigation does not support the allegation. Therefore, the complaint is determined to be "not valid".
