Technical Assistance:
22VAC131-220.A-recommendations were made regarding the documentation of core competency training.

Comments:
An unannounced monitoring inspection was conducted on March 11, 2020 from 8:30 am to 12:30 pm at the Roanoke location. Extra Special Parents, Inc. continues to provide foster care, short-term foster care and treatment foster care services. The agency reports seven (7) children in care and nine (9) approved foster homes.

During the inspection, the following activities transpired:
1. Two (2) active foster home records were reviewed (includes one newly licensed location)
2. One (1) active foster child record was reviewed.
3. Two (2) staff records were reviewed.
4. Policy and Procedures were reviewed.
5. An interview with an agency representative was conducted.

There were four (4) citations for violation of the Standards for Child Placing Agencies. An exit meeting was conducted with three agency representatives, the Co-executive Director, Director of Branch Operations and the Administrative Assistant on March 11, 2020 at 11:30 a.m. to discuss the inspection.

Violations:
Standard #: 22VAC40-131-180-D
Description: Based on an interview with Agency Representatives (AR) and based on a review of the Foster Home (FH) record for FH1, the licensee failed to conduct a minimum of three face-to-face interviews on three separate days with each applicant.

Findings:
1.Based on the documentation located in FH1, the licensee documented conducting an interview with each applicant on two separate occasions and not three separate occasions.
2.During the exit interview, AR1 acknowledged that each applicant was interviewed twice on two separate occasion and not on three separate occasions.

Standard #: 22VAC40-131-250-L
Description: Based on an interview with Agency Representatives and based on the review of the record for Foster Child (FC) #1, the licensee failed to document an interview with the child prior to the child?s placement.

Findings:
1.The record for FC1 contained no documentation of an interview with the child prior to the child?s placement.
2.During the exit interview, AR1 and AR2 presented a document titled ?Placement Narrative? with a signature date of 11/01/2019 which documents that the child ?met? the prospective foster parents 13 days prior to placement. However, this document does not document that the child was interviewed.

Standard #: 22VAC40-131-250-M
Description: Based on an interview with Agency Representatives and based on the review of the record for Foster Child (FC)1, the licensee failed to conduct a pre-placement visit for the child in the prospective home.

Findings
1.The record for FC1 contained no documentation of a pre-placement visit for the child in the prospective home prior to the placement.
2.During the exit interview, AR1 and AR2 presented a document titled ?Placement Narrative? with a signature date of 11/01/2019 which documents that the child ?met? the prospective foster parents 13 days prior to placement. However, this document does not document a pre-placement visit.
3.During the exit interview, AR3 reported that the pre-placement visit was conducted in the community when the FC1 met the prospective parents in the community 13 days prior to placement.

Standard #: 22VAC40-191-40-C-1-d
Description: Based on an interview with agency representatives (AR) and based on the review of the Foster Home (FH) record for FH1, the licensee failed to obtain a central registry, search of child abuse and neglect registry maintained by another state in which the foster home provider resided in the last five years.

Findings:
1.Based on the review of the FH record for FH1, the Certificate of Approval documented that the home was approved on 03/03/2020.
2.The sworn disclosure located in the record for FH1 documented that the foster home providers resided in another state within the last five years. However, there was no central registry, search of child abuse and neglect registry located in the record for this state.
3.During the exit interview, AR1 acknowledged that the foster home provider sworn statements documented that the foster home providers resided in this other state within the last five years. AR1 acknowledged that there was no central registry, search of child abuse and neglect registry completed for this state.
