Technical Assistance:

Technical Assistance provided regarding standard 22VAC40-131.290.J documenting adverse reactions to medications. Technical Assistance provided regarding standard 22VAC40-131.290.F documentation of medical information and ensuring follow up medical care.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on July 17, 2020 and concluded on July 22, 2020. The Therapeutic Foster Care Program Director was contacted by telephone to initiate the inspection. The agency reported 9 children in care, and 10 approved provider homes. The agency reports no new staff since the last inspection. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 2 complete child records, 3 complete provider home records and 1 staff record submitted by the agency to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Based on the review of the provider home record for foster home (FH) 3, the licensee failed to obtain a complete sworn statement or affirmation for foster parent A. Findings: The Sworn Statement or Affirmation dated 05/17/20 presented for review for foster parent A was incomplete as evidenced by no response for question number 5 on this form.Question number 5 asks the individual to disclose states in which she or he has resided during the last 5 years).
Plan of Correction:	An example form will be provided for all parents with directions to include no blank answers are allowed. Training will occur by the Director of Branch Operations to review the standards as well as checking all forms received do not have any blank areas.
