Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on February 08, 2021 and concluded on February 17, 2021. The Therapeutic Foster Care Program Director/Director of Branch Operations was contacted by email to initiate the inspection. The agency reported 12 children in care and 11 approved provider homes. The agency reports no new staff since the last inspection. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 1 complete child record, 2 complete provider home records and board officer records submitted by the agency to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-290-A
Description: Based on the review of the records presented for Foster Child (FC)-1 and based on an interview with agency representatives (AR), the licensee failed to obtain a medical examination conducted by or under the direction of a licensed physician no earlier than 90 days prior to placement.

Evidence
(1)The records presented for FC-1 did not contain a medical examination.
(2) During the inspection, the inspector requested documentation of a medical examination for FC-1 however; AR-1 acknowledged that they did not obtain a medical examination for FC-1.
(3) During the exit interview, AR-1 and AR-2 acknowledged that a medical examination was not obtained for FC-1.

Plan of Correction: Within 30 days of a new admission, in the event a medical examination was not provided, an appointment will be set up with a primary care doctor to complete the exam.
