Technical Assistance:
Discussion occurred during the exit interview regarding normalcy for foster children and ensuring clear language across all agency forms, clarity of language about storing keys/combination to firearm and ammunition lock boxes out of reach of children, how the agency assures that foster parents understand requirement to contact the agency if evacuating, and the agency's system for 24/7 foster parent support. The agency is pursuing an accreditation and policies and procedures will be revised during this process.

Comments:
An unannounced monitoring inspection was completed on 9/3/2019 from 10:00 a.m. to 5:00 p.m. and 9/4/2019 from 9:20 a.m. to 12:50 p.m. The licensee reports 47 children in foster care and 37 approved provider homes. There were three new hires since the last inspection. During this inspection five children?s, five foster home and three personnel records were reviewed. Inspection documents were prepared, signed, and left with the agency on 9/4/2019.

The Site Supervisor was available for the entrance conference and inspection. The Executive Director and Foster Home Coordinator were available during the inspection. An exit interview was conducted on 9/4/2019 from 11:00 a.m. to 11:55 a.m. with the Site Supervisor and Executive Director. The Foster Home Coordinator participated in a portion of the exit interview.

There was one citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review and interview, the agency failed to include all required elements in the Home Study for foster home 2 (FH2).

Findings:

The Home Study in the record of FH2 did not contain dated signatures of the staff completing the study or the Director or Director's designee. The finding was reviewed and discussed with agency representatives during the exit interview. Agency representatives acknowledged the home study did not contain the dated signatures.

Plan of Correction: The home study reviewed did not contain the electronic signature. There was an error in the field coding of the homestudy form, this has thus been corrected.

A homestudy addendum will be completed to add the staff signature and explanation

All HS will be reviewed to ensure there were no other missing signatures.
