Comments:

An unannounced renewal inspection was conducted on 4/29/2024 from 10:30 p.m. to 2:00. p.m. by this Licensing Specialist. Staff records were reviewed on 5/1/2024 from 10:00 to 12:00. Exit interview completed on 5/8/2024. The Licensing Specialist contacted the Director to secure electronic record access on 4/29/2024. . An entrance conference was conducted on the date of inspection. An Acknowledgment of Inspection form was completed, and exit completed and the inspection documents emailed to the provider on 5/8/2024. Note: an office location inspection was not completed during this inspection Number of children in care: 4 Number of approved provider homes: 2 Number of children?s records reviewed: 1. Number of staff records reviewed: 2. 50% of personnel records have been reviewed during the license period. \ Background checks for the Members and Managers of the Limited Liability Company were reviewed. The evidence gathered during the inspection determined non-compliance with (1) violation of applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Description:	Violation: Based on the record review for FC-1, the agency failed to demonstrate that a follow-up medical recommendation was addressed based on the medical examination report signed and dated on 10/3/2023. Findings: 1. Physical examination documentation dated 10/3/2023 in the record for FC-1 did not document a follow-up diagnostic blood test per recommendation by the physician. 2. Documentation was not made available in the electronic record. 3. The findings were discussed during the preliminary findings and exit review.
