Comments:

An unannounced monitoring inspection was completed on 7/29/2019 from 3:30 p.m. to 4:50 p.m. and on 7/30/2019 from 8:30 a.m. to 1:00 p.m. The licensee reports a total of eight children in foster care and eight approved foster homes. No new staff have been hired since the last inspection. During this inspection one child and three foster home were reviewed. The Site Director was available for the inspection and participated in the exit interview. The Site Director from the licensee's Fredericksburg office participated in the exit conference by phone. Inspection documents were completed and provided at the end of the inspection. There were three citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #:	22VAC40-131-180-J-2
Description:	Violation: Based on review of the record for FC1and interview, the agency failed to include dates with the signatures of the individual completing the home study and the licensee's executive director or designee. Findings: The Home Study of FH1 includes signatures without dates. During the exit interview the Site Director acknowledged the document did not contain dates for signatures.
Plan of Correction:	Site Director has completed an audit of all home studies to ensure there were no additional dates missing by signatures. The audited charted was corrected to include the date it was staffed, approval date, the date the approval letter was mailed.
Standard #:	22VAC40-131-250-O
Description:	Violation: Based on record review and interview, the agency failed to document all required elements in the written assessment for FC1. Findings: The written Intake Assessment for FC 1, dated 6/20/2019, did not include a list of the strengths for the child's birth family. During the exit interview, the Site Director acknowledged the document did not contain this information.
Plan of Correction:	Site Director will complete an audit of all intake/social histories and has added any necessary addendums for missing biological parental strengths.

Description:	Violation: Based on record review and interview, the agency failed to address all elements in the individualized service plan for FC1. Findings: The individualized service plan for FC1, dated 6/29/2019, did not include a comprehensive assessment of the child's nutritional status. During the exit interview the Site Director acknowledged the plan did not include the required information.
Plan of Correction:	Site Director will complete an audit of all service treatment plans and has added any necessary addendums for missing nutritional status.
