Comments:

A monitoring inspection was initiated on November 17, 2021 and concluded on November 22, 2021. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported there were 8 approved foster homes and 9 children in foster care. The inspector emailed the Executive Director a list of items required to complete the inspection. The inspector reviewed one personnel record, one child?s record, three foster home records submitted by the agency to ensure documentation was complete. Exit meeting was held with the Executive Director on November 22, 2021. Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the agency

Standard #:	22VAC40-131-330-G-5
Description:	Violation: Based on review of the record for foster child FC1 and interview with agency representative AR1, the agency failed to document that FC1 was interviewed privately during the month of September 2021. Findings: 1. FC1's record did not document a private interview in September 2021. 2. AR1 confirmed that the notes did not document a private interview during September 2021.
Plan of Correction:	Staff reported this particular incident was a typo, and he was placing another client with that name on that day and made a mistake. The file has been adjusted to ensure it has the correct names of clients.
