Comments:
An unannounced monitoring inspection was completed on July 10, 2019 from 9:45 a.m. to 2:00 p.m. The licensee reports a total of nine children in foster care and seven approved provider homes. One child's record and one provider home record was reviewed during the inspection. The team leader and case manager were available for the inspection and exit interview. The program manager participated in the exit interview via telephone which commenced at 1:30 p.m. There was one citation for violation of the Standards for Child-Placing Agencies. See the violation notice on the Department's public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation: Based on the review of the file for Foster Child (FC) FC1, the licensee failed to document a valid reason for not involving the birth parents in the development of the child?s individualized service plan, treatment plan and updates of the service plan. Findings: FC1 file does not document a valid reason for not involving the birth parents of the child in the development of the child?s individualized service plan, ongoing treatment plan and updates to the service plan. 1. The service plans in the record indicates the goal of return home. 2. The birth mother is actively involved in the child?s life as evident by ongoing supervised visitations and weekly telephone contacts with the child. 3. The birth parent?s rights have not been terminated. 4. There was no documentation in the record of the birth parent's involvement in the development of the individualized service plan and updates to the service plan. 5. During the inspection on 7-10-19 at approximately 12:05pm, Agency Representative (AR) AR1 reviewed the record and could not locate documentation of the birth mother or birth father?s involvement in the development of the child?s individualized service plan, child?s treatment plan and updates of the service plan.
