Technical Assistance:
The code of ethics and mutual responsibilities for FC1 and FC2 lists the placing agency as a party to the agreement. This agreement should be made between the licensed child-placing agency and the foster parent. When the form lists LDSS/LCPA, it is because the form is used by both public and private child-placing agencies. Please enter the information for your agency for those elements.

Comments:
An unannounced renewal inspection was completed on October 10, 2019 from approximately 9:50 a.m. to 2:00 p.m. The licensee reports eleven children in foster care and seven approved provider homes. Two children's record, one provider home record and two staff records were reviewed during the inspection. The casemanager was available for the inspection and exit interview. The program manager participated in the exit interview which commenced at 1:30 p.m. on October 10, 2019.

There were 5 citations for violation of the Standards for Child-Placing Agencies. See the violation notice on the Department's public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on a review of the record for Staff (S)S2 and interview with the program manager, the licensee failed to document the written training program as required by 22VAC40-131-150-G-2 in the record.

Findings:
The record for S2 documents that S2 was hired as a caseworker trainee in July 2019. The record for S2 did not include the written training program listing topics to be covered during the period S2 will be a trainee. During an interview with the program manager it was acknowledged that S2 was hired as a caseworker trainee.

Description: Violation: Based on review of record and interview with case manager (CM), the case record for FC2 contains a report that is not signed or dated.

Findings: The record contains a report titled, ?Goals for [FC2]?. It is not signed or dated. When interviewed, CM stated that she believed the case manager to be the author. CM did not know when it was written.

Description: Violation:
Based on a review of the record for FH1 and interview with case manager (CM) the licensee failed to list all members of the household on the face sheet.

Findings:
The re-evaluation for FH1 was completed on 9/15/18 and documents Other Household Member (OHM) OHMI as residing in the home.
The face sheet for FH1 is dated 7/26/19 and does not list OHMI as a household member. During an interview with CM, it was acknowledged that OHMI is residing in FH1.

Description: Violation: Based on review of face sheet dated 8/14/2019 and interview with CM, the facility failed to document the addresses of the siblings for FC2.

Findings:
The face sheet lists the name and phone number of the sister of FC2. According to the social history and court service plan, the sister is known to be placed with another child-placing agency. When interviewed, CM reviewed the face sheet and was not able to find the address of the sister.

Description: Violation: Based on review of the record and interview with CM, the narrative case notes do not address treatment and services provided.

Findings: According to the service plan and the interview with CM, FC2 receives counseling and in-home services. Occurrences of those services are not documented in the contact notes.
