Comments:

A renewal inspection was initiated on 9/15/2021 and concluded on 9/29/2021. The Executive Director and the Program Director was contacted by telephone to initiate the inspection. The inspector emailed the Executive Director and Program Director a list of items required to complete the remote documentation review portion of the inspection. The Executive Director reported a current census of (6) youth in placement and (5) approved foster homes. The inspector reviewed one (1) foster care home and one (1) staff records. An exit interview was conducted with the Executive Director and the Interim Program Director on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection9 The information gathered during the inspection determined that there were no non-compliance(s) with applicable standards or law, and there were no violations documented on the violation notice issued to the facility.

Description:	Based on review of the record for FH1, the licensee failed to ensure that the face sheet contained the listing of the names and dates of birth of all members of the household and the relationship of each member to the provider Findings: The face sheet provided dated 9/1/2021 did not contain the relationship between the other house member #1 and the provider. The face sheet provided dated 9/1/2021 did not list other house member #2 as a member of the household
