Comments:

An unannounced monitoring inspection was conducted on 4/12/2022 from 10:40 a.m. to 4:30 p.m. The Executive Director and Case Manager participated in the entrance conference. The inspector conducted an entrance conference regarding if there were any program or policy and procedural changes since the last inspection, and there were none reported. The licensee reported a census of Four (4) foster children in placement and Four (4) approved foster homes. During this inspection One (1) foster child, One (1) foster home was reviewed. There was One (1) Discharge foster child record reviewed. There were no new personnel since the last inspection. An exit interview was conducted with the Executive Director on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined (2) violations with applicable standards or law). The licensee has five calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Please do not post the Supplemental form. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.

Description:	Violation: Based on review of the record for FH1 and an interview with the Case Manager, the licensee failed to monitor the approved provider?s home at least one time every 90 days. Findings: The record for FH1 did not include a monitoring contact visit with the approved provider at least one time every 90 days.
