Type of inspection: Monitoring
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 10:30a.m. to 2:00 p.m. The Licensing Inspector met with Case Manager and the Supervisor via telephone at the site.
Number of approved provider homes: 8
Number of children in care: 9
The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: (1). Number of provider home records reviewed: (2) and (1) was background checks only.
Number of staff records reviewed: (2). There were no new staff hired since the last inspection. Observations by licensing inspector: posting of the license and inspection documents was observed. An entrance meeting was conducted on 3/1/2023 and the exit was discussed on 3/2/2023.
The evidence gathered during the inspection determined (3) violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary. Description: Violation:
Based on the record review, the agency failed to document a written assessment for (FC-1) within 30 days of the placement.
1) (FC-1) was admitted on 10/21/2021
2) The record indicated that the licensee?s written assessment was completed and dated on 3/7/2022.
3) The Licensing Inspector requested the written assessment for (FC-1)
4) The Licensing Specialist asked (AR-3) if the information could be located
5) The findings were discussed during the preliminary findings review and the staff acknowledged the findings.
6) The findings were reviewed during the exit interview.

Based on the record review, the agency failed to document an implement an individualized service plan and treatment plan for FC (1) within 45 days.

2) The record did not contain the comprehensive individualized service and treatment plan until 3/28/2022
3) The Licensing Inspector requested the plan of care
4) The Licensing Specialist asked (AR-3) if the information could be located
5) The findings were discussed during the preliminary findings review and the staff acknowledged the findings.

Description: Violation:
Based on record review, the agency failed to document the signature of the staff member that developed the initial plan of care service plan for FC (1).
Findings:
1) (FC-1) was admitted on 10/21/2021.
2) The initial service plan included in the record for (FC-1) written on 10/28/2021, did not include the signature of the staff individual who developed the plan as identified on the top of the plan as ( AR-2)
3) The Licensing Inspector requested the plan developed by the identified staff member at the top of the initial plan of care.
4) The Licensing Specialist asked (AR-3) if the information could be located.
5) The findings were discussed during the preliminary findings review and staff acknowledged the findings.
6) The findings were reviewed during the exit interview
