Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: The Licensing Specialist initiated a request for an inspection with the Case Manager on 9/26/2023 at 10:30 a.m. The Licensing Inspector discussed the census request and the files to be reviewed. Additional information was requested from the licensee and the exit to close the inspection was completed on 9/27/2023. Number of approved provider homes: (9) Number of children in care: (6) Number of children?s records reviewed: (1). Number of provider home records reviewed: (1) Number of staff records reviewed: (0) as the staff records and the board members were reviewed by Licensing Specialist Dawn Espelage at the Roanoke location. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. Description:	Violation: Based on a review of the adult household member?s sworn statement provided for Foster Home (FH) FH1, Individual #1 resided out of state in the last five years for which central registry background checks are required. The licensee failed to obtain the required central registry background check within 30 days of the person residing in the foster home. Findings: Individual #1 is an adult household member of FH1. Individual #1?s sworn statement dated 3/11/2023 confirmed the answer of (yes) to question #5 about residing in the state of Arizona. During the exit interview with AR-1, it was confirmed and acknowledged the absence of the central registry background for Individual #1 from the state of Arizona.
