Comments:

Type of Inspection: Monitoring On 6/20/2024 Licensing inspector initiated a monitoring inspection. The Director was contacted by telephone to initiate the monitoring inspection. The Director reported that he current census was (11). The Licensing Inspector emailed the Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (2) child record, (2) staff records, and (2) Provider home records submitted by the agency to ensure documentation was complete. Provider has not had any updates to policies and procedures since last inspection. An exit interview was conducted with the Director on 6/28/2024 via TEAMS, where findings were reviewed, technical assistance was provided and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations documented on the violation notice issued to the facility. There were no changes to facility's policies or procedures since the last inspection. Description:	Based on a review of record for foster Home (FH2), home study in record did not include the signature of the licensee?s executive director or its designee. Findings: 1. Foster Home #2, home study in record titled ?Initial Home Study narrative? was signed and dated by staff completing the home study on 5/8/23. The signature area on the home study for the licensee?s executive director or designee was blank and did not include a signature or date. 2. Inspection findings were discussed during the exit interview with AR-1 and acknowledged the absence of the required signature of the executive director or its designee.

Description:	Based on the record review for foster child #2. Social history in record was not completed within 45 days of the foster child #2 placement. Findings: 1. Foster child #2 placement occurred on 11/16/2023. 2. Social history in the record for FC #2 was completed and signed by staff on 1/31/23. 3. Inspection findings were discussed during the exit interview with AR-1 and finding were acknowledged following AR-1 review of documentation in agency?s electronic health record.
