Comments:
A renewal inspection was conducted on 5/17/2022 from 11:00 a.m. to 4:00 p.m. and 5/18/2022 from 9:15 to 11:30 p.m. The State Trainer and Case Manager participated in the entrance conference. The inspector conducted an entrance conference regarding if there were any program or policy and procedural changes since the last inspection, and there were none reported. The Licensing Inspector reviewed the posted license and the previous inspection. The Licensing Inspector discussed the renewal application and noted if there were any changes. The licensee reported a census of 15 provider homes and 15 children in care. The Licensing Inspector reviewed Two (2) foster children in placement and One (1) discharge record. The Licensing Inspector reviewed Two (2) provider homes. During this review, all board officers were reviewed, and Two (2) personnel records. An exit interview was conducted with the Executive Director and the State Trainer on the date of the inspection. Findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined (3) violations with applicable standards or law).

The licensee has five calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Please do not post the Supplemental form. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.

Description: Violation:
Based on record review and interview, the
agency failed to complete a written assessment for
Foster Child (FC1) within 30 days of
placement.

Findings:
1) The Licensing Inspector requested the written assessment for FC1
2) The Licensing Inspector interviewed Staff 1
and 3 (S1 and S3).
3) S1 and S3 reported the document was not
completed prior to the date of inspection.
4) The findings were discussed during the
preliminary findings review and S3
acknowledged the findings

agency failed to complete a Social History for
Foster Child (FC1) and Foster Child (FC2) within 45 days of
placement.

Findings:
1) The Licensing Inspector requested the Social
History for FC1 and FC2
2) The Licensing Inspector interviewed Staff 1
and 3 (S1 and S3).
3) S1 and S3 reported the document was not
completed prior to the date of inspection.
4) The inspection was conducted 333 days
following the date (FC1) was placed and 192 days following the date (FC2) was placed
5) The findings were discussed during the
preliminary findings review and S3

Description: Violation

Based on a case record review and interview, the agency failed to obtain an annual medical exam report for Foster Child (FC2)

Findings:
FC2 was placed on 6/18/2021 and the case record did not include an annual medical examination report. According to the FC2?s 90-day quarterly progress summary dated 9/14/2021, the report concluded that FC2?s last medical appointment was completed in the year 2020, this written report was not included in the file. The finding was discussed with staff S1 and S3 during the exit interview.
