Technical Assistance:

Technical assistance was provided to the licensee regarding regulation 22VAC40-131-340 regarding the implementation of measurable objectives and strategies.

Comments:

Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 10/25/2022 from 10:55 to 4:00 and on 10/26/2022 from 10:00 a.m. to 2:00p.m. The Licensing Inspector met with the Clinical Supervisor and State Trainer during the entrance interview. The Inspector spoke via phone to the Treatment Foster Care Director. Number of children in care: 23 Number of approved provider homes: 16 Number of children?s records reviewed: 3. Number of provider home records reviewed: 2. Number of staff records reviewed: 3 Observations by the licensing inspector: positing of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection with the Executive Director, Treatment Foster Care Director, and the State Trainer. The evidence gathered during the inspection determined (3) violations with applicable standard(s) or law. Description:	Based on the review of the staff records presented for (P1), and based on an interview with Agency Representatives (AR), the licensee failed to provide a job description for the Clinical Supervisor. Findings: 1. Based on the review of the personnel record for (P1), the individual?s date of hire as the clinical supervisor was 10/17/2022. There was no evidence of a documented job description at the time of the position. 2. During the exit interview AR-2 and AR-3 acknowledged the absence of the job description.

Description:	Based on the review of the records presented for Foster Child (FC)-1 and (FC)-2, and based on an interview with Agency Representatives (AR), the licensee failed to document the child?s record of immunizations since the last examination. Findings: 1. Based on the review of the health examination record for FC1 and FC2, there was no record of immunizations the child has received. 2. During the exit interview AR-2 and AR-3 acknowledged the absence of the children?s immunization records.

Description:	Based on a review of the background check requirements for staff member (S1), the licensee obtained the required background checks for the staff member which was dated more than 90 days prior to the date of employment. Findings: Staff member (S1) was hired on 6/2/2022. The criminal history record for the employee was dated and accepted on 1/4/2022. During the exit interview with AR-2 and AR-3 acknowledged the absence of the updated criminal history record.
