Description:	Based on the review of the records presented for Foster Child (FC)-1 and based on an interview with Agency Representatives (AR), the licensee failed to document that the social history was completed within 45 days from admission. 1. Based on the review of the social history documentation, FC1 was admitted on 2/2/2023. The date of completion on the social history documentation was 4/26/2023. The documentation was signed on 5/25/2023. 2. During the exit interview, it was acknowledged that the social history documentation was not up to date.
