Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 12/4/2023 from 10:34a.m to 4:00p.m.

Observations by the licensing inspector: positing of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection.

Description: Based on the review of the records presented for Foster Child (FC)-2 and based on an interview with Agency Representatives (AR), the licensee failed to document the date the social history was completed.
Findings:
1. Based on the review of the social history for FC1, the documentation did not contain the date of completion.
2. During the exit interview AR-1 and AR-3 acknowledged the absence of the date on this documentation.

Description: Based on the review of the records presented for Foster Child (FC)-3 and based on an interview with Agency Representatives (AR), the licensee failed to document the child?s
evidence of freedom from communicable disease, including tuberculosis.

Findings:1. Based on the physician visit summary for FC3 dated 10/23/2023, there was no evidence that the child was free from communicable disease, including tuberculosis.
2. During the exit interview AR-1 and AR-3 acknowledged the absence of this evidence.

Description: Based on the review of the records presented for Foster Child (FC)-1 and based on an interview with Agency Representatives (AR), the licensee failed to document the child?s follow-up medical appointment.
Findings:
1. Based on the review of the record and physician visit summary for FC1 dated 11/29/2022, the child was scheduled for a follow-up medical appointment on 5/29/2023. There was no documentation that this arrangement was made.
2. During the exit interview AR-1 and AR-3 acknowledged the absence of this documentation.

Description: Based on the review of the records presented for Foster Child (FC)-2 and based on an interview with Agency Representatives (AR), the licensee failed to evaluate and update the comprehensive treatment and service plan for the upcoming year.

Findings:

1. Based on a review of the record for FC 2, the last comprehensive treatment service review was dated 10/24/2022, the licensee failed to document the completion of the annual comprehensive treatment service plan review for 10/24/2023.

2. During the exit interview AR-1 and AR-3 acknowledged the absence of this documentation.
