Comments:

Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 2/28/2024 from 10:00a.m to 4:00p.m. Staff records were sent remotely on 2/28/2024 and completed on 3/1/2024. Technical assistance was provided regarding the letter of approval sent to certified provider homes. Number of children in care: 14 Number of approved provider homes: 13 Number of children?s records reviewed: 1. Number of discharge records reviewed: 1 Number of provider home records reviewed: 2. Number of staff records reviewed: 2 Observations by the licensing inspector: positing of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection. The evidence gathered during the inspection determined (2) violations with applicable standard(s) or law.
