Provider Homes Children's Services Organization and Administration Additional Requirements for Specific Programs

Comments:

An unannounced monitoring inspection was completed on September 19, 2019 from 12:50 p.m. to 3:45 p.m. The licensee reports 5 children in foster care and 5 approved provider homes. During this inspection, one child record and one provider home record was reviewed. The state director was available for the inspection and exit interview. The program manager participated in the exit interview via telephone which commenced at 3:05 p.m. on September 19, 2019. An acknowledgment form was signed by the state director and left at the end of the inspection. There were five citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department's public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #:	22VAC40-131-250-L
Description:	Violation: Based on a review of the record for FC1 and interview with the state director, the licensee failed to ensure the child was interviewed prior to placement. Findings: FC1 was placed with the licensee on 5/16/19. The record documents a referral for placement dated 4/25/19. The written assessment completed by the licensee on 5/23/19 documents, "no interview" with the child. The record does not document a valid reason why the child was not interviewed prior to placement. During an interview with the state director it was acknowledged that FC1 should have been interviewed prior to placement.

Standard #:	22VAC40-131-250-O
Description:	Violation: Based on a review of the record for Foster child (FC) FC1 and interview with the state director, the licensee failed to ensure the written assessment contained all of the elements required by 22VAC40-131-250-G. Findings: FC1 was placed with the licensee on 5/6/19. The written assessment was completed on 5/23/219 and fails to include the names of the persons involved in staffing FC1's case. During an interview with the state director, it was acknowledged that the written assessment did not meet the standards.

Description:	Violation: Based on a review of the record for FC1 and interview with the state director, the licensee failed to ensure two face to face contacts were made with FC1. Findings: The record for FC1 documents that FC1 is receiving treatment foster care services. The record documents a face to face contact with FC1 on 7/10/19. The record also documents a contact note marked as"face to face" on 7/16/19 however, the details of the contact note made on 7/16/19 does not reflect face to face contact was made with FC1. During an interview with the state director it was acknowledged that the 7/16/19 contact note does not document face to face contact was made with FC1.

Standard #:	22VAC40-131-340-E-1
Description:	Violation: Based on a review of the record for FC1 and interview with the state director, the licensee failed to include a comprehensive assessment of the child's dental needs. Findings: The individualized treatment plan in the record for FC1 was completed on 6/19/19. The plan documents the following as an assessment of FC1's dental needs, " most recent dental was". During an interview with the state director, it was acknowledged that individualized treatment plan failed to document an assessment of FC1's dental needs.

Description:	Violation: Based on a review of the record for FC1 and interview with the state director, the licensee failed to document a written summary report. Findings: FC1 was placed on 5/6/19. The first written progress summary report was due no later than 8/6/19. At the time of review the state director and program manager were not able to provide a progress report for FC1.
