Technical Assistance:

The Licensee was provided technical assistance regarding including page numbers in headers on documents such as the home study, service plans, and discharge summaries.

Comments:

A renewal inspection was conducted on 5/2/2022 from 11:45 a.m. to 4:00 p.m. and 5/3/2022 from 9:15 to 2:00 p.m. The Asst. Director and Office Manager participated in the entrance conference. The inspector conducted an entrance conference regarding if there were any program or policy and procedural changes since the last inspection, and there were none reported. The Licensing Inspector reviewed the posted license and the previous inspection. The Licensing Inspector reviewed storage keeping for case files. The licensee reported a census of 48 provider homes and 16 children in care. The Licensing Inspector reviewed Two (2) foster children in placement and One (1) discharge record. The Licensing Inspector reviewed Four (4) provider homes and One (1) closed provider home. During this review, all board officers were reviewed, and Six (6) personnel records. An exit interview was conducted with the Executive Director, Asst. Director for the Charlottesville location, and the Admissions Coordinator on the date of the inspection. The findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined (2) violations with applicable standards or law). The licensee has five calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Please do not post the Supplemental form. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.

Description:	Violation: Based on a review of the record for P1 and an interview with the Executive Director, the licensee failed to document an annual performance evaluation. Findings: The personnel record for P1 did not include an annual performance evaluation.

Description:	Violation: Based on a review of the record for FH1 and an interview with the Case Manager, the licensee failed to document that the home complies with the standards for the home as required by 22VAC40-131-190. Findings: The record for FH1 contains a document titled, "Teaching Parent Home Evaluation Study?, and in the section titled, "Physical Environment", the foster parent documents the presence of household pets which include two cats. The home study does not document that the applicant maintains documentation that household pets receive tests, inoculations, and licenses as required by law.
