Technical assistance was provided for 22VAC40-131-180 (Home Study Requirement) The Licensee should ensure that the reference signature page is signed.

Comments:

Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 1/23/2023 from 10:30 a.m. to 4:00p.m. and on 1/24/2023 from 9:30 a.m. to 1:00p.m. The Licensing Inspector spoke with the Regional Director and Office Assistant upon arriving at the site to initiate the inspection. The acknowledgement of inspection form was sign by the Regional Director and the exit interview was completed on 1/24/2023 at 1:00 p.m. There were five new staff at the location during this inspection. An interview was conducted with (2) interns. Number of approved provider homes: (30) Number of children in care: (22) Number of children?s records reviewed: (2) and (1) Discharged child record. Number of provider home records reviewed: (3), (1) closed provider home and (2) background checks only. Number of staff records reviewed: (7). Observations by licensing inspector: posting of the license and inspection documents was observed. There were no major changes made to the policies or procedures since the last inspection. The Licensing Inspector interviewed the (2) interns regarding job duties and responsibilities as interns, training, and supervision. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. Description:	Based on the review of the records presented for Foster Child (FC-2) and based on an interview with Agency Representatives (AR), the licensee failed to develop an individualized service plan with goals and objectives to meet the child?s needs that include specific independent living services to be provided to assist the child in meeting his goals for a youth 14 years of age or older. Findings: 1. Based on the review of the individualized service plan for (FC-2), there were no independent living goals or objectives listed on the plan. 2. Based on the review of the individualized service plan for (FC-2), the plan did not list goals and objectives to meet the child?s needs that include specific independent living services. 3. During the exit interview AR-1 confirmed the youth?s age of 14 and acknowledged the absence of an individualized service plan that documented goals and objectives to meet the child?s needs that include specific independent living services
