Comments:
An unannounced monitoring inspection was conducted on 11/19/2019 from 9:00 a.m. to 2:20 p.m. and 11/20/2019 from 9:00 a.m. to 11:20 a.m. The licensee reports fifteen children in foster care and 36 approved provider homes. There were three new hires since the last inspection. During this inspection two foster child, eight foster home, and three personnel records were reviewed. The Program Supervisor and a Family Consultant participated in the entrance conference. The Program Supervisor and Quality Assurance Coordinator/Licensing Specialist remained available during the inspection to answer questions. The Human Resources Manager was available during the inspection on 11/20/2019.

The Program Supervisor, Quality Assurance/Licensing Specialist, Human Resources Manager, Chief Operating Officer, Teaching Parent Coordinator, and Admissions Coordinator participated in the exit interview that was conducted on 11/20/2019 from 4:00 p.m. to 4:55 p.m. Acknowledgement of Inspection forms were signed and left with the licensee on each inspection date.

There were two citations for violations of the Standards for Child-Placing Agencies.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on a review of personnel records for employees P1, P2, and P3 the agency failed to indicate a received date on all material received.

Findings:

The Sworn Disclosures for P1, P2, and P3 were not notated or stamped to indicate the date documents were received by the agency. The finding was discussed with agency staff during the exit interview. Staff 4 (S4) reviewed the documents and acknowledged the finding.

Plan of Correction: The Human Resources Manager, Business Office Manager, and Licensing Supervisor have reviewed this violation. This review led to an increased understanding of the licensing expectations regarding date-stamping incoming documents. The team acknowledged that current processes are working well but that the addition of date stamps are needed to meet the expectations of licensing standards.
People Places' business office has a well developed procedure for date stamping incoming documentation. The services programs have decided to utilize the business office's procedure for date stamping incoming documentation.

Description: Violation

Based on record review and interview, the agency failed to obtain an annual medical exam report that addressed the hearing for Foster Child 1 (FC1).

Findings:

FC1 received an annual physical exam on 2/8/2019. The written report for the exam did not include a notation about the child's auditory acuity. The finding was discussed with agency staff during the exit interview. Staff 1 (S1) reviewed the physical exam report and acknowledged the finding.

Plan of Correction: The Program Supervisor, Family Consultants, and Licensing Supervisor have reviewed this violation. Program staff had already identified that physicians were failing to address the hearing of infants entering foster care on the annual medical exam report. Program staff had already revised the current annual medical exam paperwork. The feeling of program staff is that these revisions sufficiently address this violation and this will not be an ongoing issue.
