22VAC40-191 (BC) -60 B Licensee shall ensure that the background checks sworn statement form contains the name of the agency on the form document.

Comments:

Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 12/20/2022 from 10:30 a.m. to 3:00p.m. and on 12/21/2022 from 9:00 a.m. to 1:00p.m. The Licensing Inspector spoke with the Program Supervisor and the Family Consultant upon arriving at the site. The Licensing Inspector met with the Program Supervisor and HR Director to initiate the inspection. The acknowledgement of inspection form was left with the Program Supervisor and the exit interview was scheduled and completed on 12/21/2022 at 1:00 p.m. There were four new staff at the location during this inspection. An interview was conducted with a family consultant employee. Number of approved provider homes: (29) Number of children in care: (14) Number of children?s records reviewed: (3). Number of provider home records reviewed: (3) and (2) background checks only. Number of staff records reviewed: (4). Observations by licensing inspector: posting of the license and inspection documents was observed. There were no major changes made to the policies or procedures since the last inspection. The Licensing Inspector interviewed the (1) family consultant employee (AR-3) regarding job duties and responsibilities related to the standards. The evidence gathered during the inspection determined (0) violations with applicable standard(s) or law. The License Specialist offered technical assistance to the licensee regarding 22VAC40-191 (BC) 60 B regarding background checks. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
