Comments:

An unannounced monitoring inspection was completed on 12/05/2019 from 9:40 a.m. to 2:35 p.m. The licensee reports 15 children in foster care and 28 approved provider homes. During this inspection two foster child and three foster home records were reviewed. An acknowledgment of inspection was completed and left with the agency at the end of the inspection. The Executive Director participated in the entrance conference, was available during the inspection, and participated in the exit interview. Two Treatment Foster Care Caseworkers also participated in the exit interview. The exit interview was conducted from 2:15 p.m. to 2:35 p.m. There was one citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on record review, the agency failed to include all required elements in the foster home agreement for a placement in Foster Home 1 (FH1). Findings: The case record for FH1 contained a Foster Home Agreement executed 9/30/2019. The agreement did not include current language approved by the State Board to address Reasonable and Prudent Parenting. The finding was discussed during the exit interview. Staff member S1 reviewed the agreement and acknowledged it is not the currently approved version.
Plan of Correction:	All old copies of the outdated form have been removed from the shared drive and filing cabinet. The correct agreement will be signed by the foster family.
