Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced mandated monitoring inspection was conducted on 5/10/2021. The Executive Director responded to the notice of inspection, provided records, and remained available during the inspection. The agency reported 14 children in placement and 31 approved provider homes. The staff listed included no new staff since the last inspection. The inspector reviewed two (2) child and seven (4) provider home records. Background checks for board officers were reviewed. The Executive Director and three (3) Treatment Foster Care workers participated in the exit interview. There was one citation for a violation of the Standards for Licensed Child Placing Agencies.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review for Foster Home 1 (FH1) and interviews, the agency failed to consider all information received about applicant 1 (A1) prior to making a decision on the applicant.

Findings:

1) The record for FH1 included a driving record report documenting A1 received a Virginia driver?s license in 2017 and surrendered a license from another state at that time.
2) The Sworn Disclosure statement for A1 documented the applicant had not resided in another state during the 5 years prior to being approved.
3) A child protective services search from the other state was not obtained.
4) The home study for FH1 documented A1 began employment in Virginia 2015 and has remained employed in Virginia since that time.
5) During the exit interview staff 1 and 2 were interviewed regarding the information in the driving record report and state of residence for A1.
6) Information in the home study and from interviews supports that A1 had not resided in another state during the five years prior to approval.
7) S1 acknowledged the information on the driving record had been overlooked and not considered. S1 acknowledged the findings.

Plan of Correction: Executive Director will train staff to thoroughly research all information received on a foster parent applicant, including DMV records.
