Comments:

An unannounced monitoring inspection was completed on 12/11/2019 from 8:30 a.m. to 1:15 p.m. The licensee reports 19 children in foster care and 25 approved provider homes. During this inspection two foster child and four foster home records were reviewed. An acknowledgment of inspection form was completed, signed, and left with the agency. The Program Administrator participated in the entrance conference, was available during the inspection, and participated in the exit interview. The exit interview was conducted from 12:45 p.m. to 1:15 p.m. There were two citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #:	22VAC40-131-180-M
Description:	Violation: Based on record reviews, the agency failed to consider all information received prior to making an approval decision for Foster Home 2 and 4 (FH2 and FH4). Findings: The Division of Motor Vehicle (DMV) report for applicant 1 (A1) in FH2 documents driving infractions and a negative driving point balance. Additional information regarding driving history was reported by A1 in another document. The DMV reports for both applicants in FH4 document driving infractions. The home studies for FH2 and FH4 do not address this information and how it was considered in the decision on the applicants. The findings were discussed with agency staff member S1 during the exit interview. S1 acknowledged the findings.
Plan of Correction:	Program Administrator and Executive Director will insure that TFC workers consider all information received about an applicant prior to making decision on applicant. An addendum will be written to address family's DMV records with each identified applicant.
Standard #:	22VAC40-131-280-C-1
Description:	Violation: Based on record review, the agency failed to include all required elements in the foster home agreement for a placement in Foster Home 2 (FH2). Findings: The case record for FH2 contained a Foster Home Agreement executed 9/17/2019. The agreement did not include current language approved by the State Board to address Reasonable and Prudent Parenting. The finding was discussed during the exit interview. Staff member S1 acknowledged the finding.
Plan of Correction:	All old copies of the outdated form will be removed from shared drive. The correct agreement will be signed by resource parents and placed in the family file.
