Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 3/18/2024 from 10:30 a.m. to 3:00 p.m. The Licensing Inspector met with the Executive Director at the site to initiate the inspection. Description:	Violation: Based on the record review for FC-3, the agency failed to demonstrate follow-up medical evaluation as identified on the medical examination report signed and dated on 8/23/2023. Findings: 1. Physical examination documentation dated 8/23/2023 in the record for FC-3 requested a chest clinic evaluation as recommended by the physician. 2. The record did not contain documentation of this recommendation for follow-up by the physician. 3. The findings were discussed during the preliminary findings and exit interview.
