Comments:

An unannounced monitoring inspection was conducted on December 6, 2019 from 9:34 a.m. to 12:04 p.m. The agency continues to provide independent living arrangements (ILA). At the time of the inspection, the agency reports sixteen (16) youth placed in ILA. During this inspection, one personnel record and and two youth records were reviewed. An exit meeting was conducted with Staff S1 on December 6, 2019 prior to leaving the agency. An acknowledgment form was signed.
