Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 5/15/2023 from 9:30a.m. to 4.00 p.m. The Licensing Inspector met with the Family Care Support Specialist to initiate the inspection and concluded an exit with the Director of Quality , and the Chief Quality Officer. Number of children?s discharge records (1) Number of provider home records reviewed: (1). Number of staff records reviewed: (5). Observations by licensing inspector: posting of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection. There were no major changes made to the policies or procedures since the last inspection. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. Standard #:	22VAC40-131-290-F
Description:	Based on the review of the record for FC1 and an interview with Agency Representatives (AR), the licensee failed to make arrangements for FC1 to receive recommended dental follow up care and treatment. Findings: 1.The dental exam located in the record, dated 8/31/2022 recommended follow up dental orthodontic care, However, this care was not documented as completed, although the case note described the care as completed on 3/2/2023. There was no evidence of follow up regarding this care in the record. 2. During the exit interview, AR4 acknowledged that there was no evidence of follow up orthodontic care and the results.
Plan of Correction:	Program Manager will work with staff regarding the requirements for the dental exam to be obtained and filed within 30 days of the exam. Staff will also go directly to the provider with a release to produce required documents.
